Comments not showing up when issuing item

I am trying to place a comment in a field when issuing items.  This is Check in/out 4.0.  Looking at the DB it seems that the "comments" section of the "Issues" subform should be putting information into the "Comments" field of the "History" table.  This is not happening.  All comments are disappearing.  the field looks like it is mapped correctly.
